At Sound Mind Counseling Services, our Adult Day Health (ADH) program is designed to serve adults with chronic health conditions, disabilities, or behavioral health needs. Located in Snellville, Georgia, this weekday program offers daily structure, supervision, and enrichment to promote independence and improve quality of life.
Whether your loved one needs daily support, social interaction, or access to health and wellness services, our ADH program provides a compassionate environment that supports their unique needs.
Supervised Daytime Care: A Safe and structured setting with trained staff present throughout the day.
Life Skills Support: Activities that support independence, such as cooking, hygiene, and communication.
Health Monitoring: Light wellness checks, medication reminders, and support from licensed professionals.
Enrichment & Recreation: Daily activities include games, creative expression, fitness, and group engagement.
Nutritional Meals: Healthy breakfast, lunch, and snacks are served each day.
Transportation Assistance: Limited local transport options are available upon request.

Participants must meet the following criteria:
Be 21 years of age or older
Have a qualifying mental health condition, disability, or chronic illness
Be able to engage in a structured daytime setting with support
Be eligible for Medicaid waiver, Medicare Part B, or willing to pay privately
Complete an intake assessment with our team prior to enrollment
We welcome referrals from:
Case managers and care coordinators
Hospitals and primary care providers
Mental health providers and agencies
Community organizations and social workers
Our team is committed to making the referral process seamless with timely follow-ups, eligibility verification, and collaborative communication.
