Overview:
The LPN supports the Adult Day Health program by providing hands-on clinical care to participants. Under the supervision of the RN, the LPN will assist with medication reminders, health monitoring, and documentation of care.
Key Responsibilities:
Monitor client health status and vital signs
Provide direct care and medication prompts
Document health services in client charts
Communicate changes to the supervising RN
Support the daily activities of the ADH center
Qualifications:
Current LPN license in Georgia
Experience working with aging adults or behavioral health clients preferred
Must be reliable, compassionate, and detail-oriented
Location: Snellville, GA | Position Type: Part-time or Full-time
Overview:
Our therapist will deliver behavioral counseling services in an intensive outpatient (IOP) setting. Responsibilities include conducting group and individual therapy, completing assessments, developing treatment plans, and contributing to interdisciplinary care.
Key Responsibilities:
Provide trauma-informed therapy services
Facilitate therapeutic groups 3–5 times per week
Conduct assessments and develop recovery-oriented treatment plans
Collaborate with the clinical team and case managers
Maintain documentation in compliance with DBHDD and Medicaid standards
Qualifications:
Active Georgia license (LPC, LCSW, or LMFT)
Experience in IOP, PHP, or community-based mental health
Knowledge of CBT, DBT, and trauma recovery techniques
Location: Snellville, GA | Position Type: Full-time
Overview:
Our Case Manager/Social Worker helps bridge gaps in client care by connecting them with housing, employment, food security, and other support services. The role is vital in reducing barriers and improving client outcomes across our programs.
Key Responsibilities:
Conduct biopsychosocial assessments
Link clients to community resources and track referrals
Collaborate with therapists and nurses to support client goals
Document services accurately and timely manner
Support discharge and transition planning
Qualifications:
Bachelor’s or Master’s degree in Social Work or Human Services
Minimum 1 year of experience in mental health or adult services
LMSW preferred but not required
Familiarity with Medicaid and community referrals
